Medicare CDM Plan

Medicare rebates may be available where weight intersects with a chronic condition.

If you’re managing weight in the context of a chronic condition — such as type 2 diabetes, high cholesterol or heart health — you may be eligible for Medicare rebates on dietitian appointments through a Chronic Disease
Management (CDM) Plan from your GP.

Under this scheme, eligible clients can access rebated allied health sessions per calendar year — which can include dietitian consultations.

Eligibility is decided by your GP. Private health funds also

Common questions we hear from clients considering working with a dietitian on their weight.

Do I need a referral to see a weight management dietitian?

No referral is required to book a private appointment. If you would like to access Medicare rebates, you will need a referral from your GP under a Chronic Disease Management (CDM) Plan. Your GP can advise whether you are eligible.

Eligible clients with certain chronic conditions may be able to access Medicare rebates through a Chronic Disease Management (CDM) Plan from their GP. Many private health funds also offer rebates for dietitian consultations under extras cover. Rebates vary, so check with your fund to confirm your level of cover.

Not in the traditional sense. We focus on building sustainable eating patterns and behaviours rather than handing you a rigid daily meal plan. Where structure is helpful, we may provide meal frameworks, examples and ideas that you can adapt to your preferences, schedule and household.

Sustainable change takes time, and the pace looks different for every person. Rather than focusing on a specific timeline or number on the scale, we focus on building habits, behaviours and outcomes that are meaningful to you and realistic to maintain. Your dietitian will discuss realistic expectations at your first appointment.

Yes. We regularly support clients with a history of disordered eating who want non-restrictive, compassionate weight care. Where appropriate, we work as part of a broader multidisciplinary team, including psychologists and GPs.

Yes. We regularly collaborate with GPs, endocrinologists, bariatric surgeons and other allied health providers, and can provide updates or correspondence to your medical team where appropriate, with your consent.
