Medicare rebates through a Chronic Disease Management Plan.
If you live with a chronic condition, you may be eligible for Medicare rebates on dietitian appointments through a Chronic Disease Management (CDM) Plan from your GP.
Under this scheme, eligible clients can access rebated allied health sessions per calendar year — which can include dietitian consultations.

No referral is required to book a private appointment. However, if you want to access Medicare rebates, you’ll need a referral from your GP under a Chronic Disease Management (CDM) Plan. Your GP can advise whether you’re eligible.

Not in most cases. Our approach is built around realistic, sustainable changes rather than complete elimination. Where specific foods do need to be reduced or adjusted, we work with you to find versions, swaps and strategies that still fit your preferences and lifestyle.
Yes. We regularly collaborate with GPs, endocrinologists, cardiologists, nephrologists and other specialists, and can provide updates or correspondence to your medical team where appropriate, with your consent.
Frequency depends on your condition, your goals and where you are in your management. Many clients start with an initial consultation followed by regular reviews every four to six weeks, then move to less frequent maintenance appointments once their plan is established. Your dietitian will recommend a schedule that suits your situation.
